 Hospitals: The Discharge Process

If you have the unfortunate situation of having a parent or older adult relative that is hospitalized, there are a few important issues to be aware of.  The length of their stay is based upon Medicare Guidelines for Hospital Admissions.  
Once their situation has improved and they no longer have a fever, need for IV antibiotics or have other critical care needs, they will likely be discharged soon there after. Often, there isn't much of a notice that discharge is imminent. 

If the patient has a need for on-going rehab beyond the therapy they received at the hospital, Medicare will cover 2 weeks of home therapy or out-patient therapies and a couple of months of therapy at a Skilled Nursing Facility, after a 3 day hospital stay.

The discharging doctor will recommend which option is best for the patient, given their particular situation. The person must participate fully and be making progress in therapy, for Medicare to cover the cost. Relatives are able to give their input and preference to the doctor and case manager as to whether in home care or a placement facility is best for the patient and family. If they choose to stay in their own home, the questions below may guide you.

Important questions to consider about
after-care for your loved-one are.
.Will your parent or loved-one be able to safely manage on their own at home?
.Would they benefit from an in-home safety evaluation?
.Will there be family or other competent help available should they need the support?
.Is their home able to accomodate a wheel chair, walker or other medical device?
.Would they benefit from in-home care or a Case Manager who could manage their needs?

Barbara integrates 25 years of clinical experience in manual therapy techniques, patient education and a love of movement.  She has been in private practice in Petaluma since 2006.  She creates a personalized program for each client with treatment techniques; therapeutic exercises, a safety assessment, postural and movement education, progressive rehabilitation, breathing exercises and “hands on” treatment to help you to attain your goals.  She looks forward to working with you!
