Q: What Does a Care Manager Do?

A: 
Private Care Managers are master's level professionals
who specialize
in assessing the needs of the elderly, or persons
with an illness. They work
to arrange for patient's needs and they
provide assistance and oversight of
medical issues.

Private Care Managers have training in elder care, social work,

health care, and counseling. They also have extensive knowledge
about medical
resources and they are concerned with quality
standards of patient care.

Additionally, they have relationships with agencies that provide
resources and
services in the community and make
recommendations and referrals.



Q: How does a Caregiver and a professional
      Care Manager differ?

A: A Caregiver provides direct personal care and companionship.
Also, a Caregiver is able to do light housekeeping, prepare meals
and assist in daily health routines for patients in the home,
or at an Assisted Living Facility.


A Care Manager is a professional (nurse, counselor, social worker,
gerontologist)
who is trained to manage a range of health,
psychological, emotional, functional,
legal and financial issues.

Care Manager’s may oversee the recruitment, training
and supervision
of Caregivers, to ensure their work is in keeping with the standards

of patient care. Care Managers also intervene with Caregiver issues
and help with
challenges or crises.

Care Managers also have an intricate understanding of the
health
care field and have access to resources and services.

      Care Manager?

A: The help of a Professional Care Manager allows relief from total

Q: How are services paid?

A:  Heart & Soul Care Management services are billed on an hourly
and fee per service basis. Home care and Care Management services
are priced separately. Fee schedules are available upon request.

Heart & Soul Care will coordinate billing for long-term care insurance
for Home Health Care, Medical Advocacy & Care Management services.
