»  Initial Interview - $90/hr

»  Functional & Home Safety Assessment - $90/hr

»  Comprehensive Case Management - $90/hr

»  Home Health Care / Caregiver Assistance
   

            

»  HOURLY RATE: $22/HR

»  LIVE-IN RATE:  $225/DAY (24hr shift)  

»  2 HR BATH VISIT $75

»  4 HR's:  $25/hr

»  12 HOUR OVER-NIGHT $160/shift

»  Weekly phone check-in + 1 visit - $75/month

»  Long-Term Care Planning - $90/hr

» Written Care Plan and Recommendations - $190 - $390
      Simple overview plans & in-depth long-term plans

» Implementation of Care Plan Referrals   
      (Prices vary per individual)

» Oversight of In-Home Caregivers $100 - $200/month          
      (price varies-pls inquire)

» 24 hr. On-Call - $100 - 200/month  (price varies-pls inquire)

»  Medical Advocacy ~ Accompany to Medical Appointments  $50/hr

»  Assistance with Advance Health Directives - $90/hr

» Bill Pay and help with managing applications, forms and mail 
      $50/hr
