ABOUT OUR PRODUCTS AND SERVICES POLICY

All products and services are at-will and offered on a fee
per-service basis. Individual Care Management services are
provided by the sole independent consulting service owner and
operator of Heart & Soul Care Management sole proprietorship.

Community services and referrals for agency support services are
provided for the sole purpose of meeting the individual and specific
needs of the recipient and are accepted or declined at will
by the intended recipient.

Referrals are not intended for definitive recommendations, rather
they are suggestions, based upon the owner's experience, for
services that may enhance needs of recipients.

Products are provided to enhance the needs of the recipients.
Any dissatisfaction of product or services are of care and concern
of Heart & Soul Care Management, and it's owner who will evaluate
all complaints based on the company's Standards of Care and when
applicable and justified, return fees for services or products that do
not meet the expectations of recipient. All products and services are
screened by owner who is not liable and has no ultimate control over
the delivery or quality of the products and services provided by
outside contractors or vendors, not hired by, owned or operated by
Heart & Soul or it's agents and owner.
